Plan To Protect Policy

Dear Beloved Servants:
We pray that you are doing well. The Church has purchased a new insurance plan for the mutual protection of both the Church and its servants. In order for us to be covered by this insurance plan, we need the following from all the servants:
  • STEP 1 - Complete a Vulnerable Sector Check (VSC)
  • STEP 2 - Review Information about the "Plan to Protect Policy"
  • STEP 3 - Volunteer Application Form 
  • STEP 4 - Two Reference Forms

The Church Fathers, Board & the Plan to Protect Committee
STEP 1 - Complete a Vulnerable Sector Check (VSC)
  • please complete the VSC that corresponds with your primary place of residence
  • if you need a letter stating that you are applying for a volunteer position with the Church, please speak to your head servant or email: insurance@stmarkstoronto.ca
  • Once you have received confirmation of completion of the VSC, please send a copy of it to insurance@stmarkstoronto.ca

If you have already completed a VSC outside of the church (e.g. school board), please skip this step and go to step 2.
STEP 2 - Review Information about the "Plan to Protect Policy"
- Please read Form 1
- Complete & sign the "Covenant of Care" (Form 2)
- Please send completed form to: insurance@stmarkstoronto.ca
STEP 3 - Volunteer Application Form 
- Please complete the "Application Form for Potential Servants Working with Children, Youth, and Vulnerable Adults" (Form 3)
- Please send completed form to: insurance@stmarkstoronto.ca
STEP 4 - Two Reference Forms
- Please ask two adults who know you well to each complete a Reference Form for you (please give Form 4 to your references)
- Please send completed Reference Forms to: insurance@stmarkstoronto.ca
